This form is only to be filled out by a representative in the Human Resources department for incoming or outgoing staff and faculty. Doing so provides the required authorization informing Technology Services that all of the requirements for a personnel change have been met. Any submissions generated by anyone other than HR will be discarded.
Upon receipt of this HR Change Authorization, the supervisor of the employee will be asked to complete the New Hire Form or Outgoing Employee Form. Please note that new hire forms must be submitted two weeks prior to any employee’s start date to ensure proper setup for the employee’s first day.
